GLP-1 Side Effects: What to Expect and How to Manage Them

Starting a new medication always comes with questions about side effects. With GLP-1 receptor agonist medications, most patients experience some adjustment period. Understanding what to expect can help you feel more prepared and know when to reach out to your provider.

The most common side effects

Gastrointestinal effects are by far the most frequently reported. Nausea is the most common, particularly in the first two to four weeks of treatment or after a dosage increase. This is because GLP-1 medications slow gastric emptying, which your body needs time to adjust to.

  • Nausea — typically mild and most common in weeks 1-4
  • Reduced appetite — this is also the intended therapeutic effect
  • Occasional digestive discomfort or changes in bowel habits
  • Less commonly: headache, fatigue, or dizziness

How long do side effects last?

For most patients, the initial adjustment period lasts two to four weeks. Side effects tend to be most noticeable during the first week or two, then gradually diminish as your body adapts. Dosage increases may temporarily bring back mild symptoms, but these also tend to resolve more quickly with each adjustment.

Practical tips for managing nausea

  • Eat smaller, more frequent meals rather than large portions
  • Avoid high-fat or greasy foods, especially in the first few weeks
  • Stay well hydrated throughout the day
  • Eat slowly and stop when you begin to feel full
  • Keep bland foods available (crackers, toast, rice) for days when nausea is more noticeable
  • Take your injection at a consistent time each week

When to contact your provider

While most side effects are mild and temporary, there are situations where you should reach out to your care team. Contact your provider if nausea persists beyond four weeks without improvement, if you experience persistent vomiting, if you notice unusual pain or swelling, or if side effects are significantly affecting your daily life.

Seek emergency medical attention immediately if you experience severe abdominal pain, signs of an allergic reaction (difficulty breathing, swelling of face or throat), or symptoms of pancreatitis. Call 911 — do not wait for a telehealth appointment.

The role of gradual dose titration

One of the most effective strategies for minimizing side effects is gradual dose titration. This means starting at the lowest available dose and slowly increasing over several weeks or months. Your provider will create a titration schedule tailored to how your body responds.

This approach gives your body time to adapt at each dose level before moving to the next, which significantly reduces the severity and duration of side effects for most patients.
